Tooth Whitening or Bleaching is a very popular and effective way of drastically improving a smile! The Canadian Dental Association states that tooth bleaching is a generally safe and effective method to improve tooth esthetics, provided it is undertaken under a dentist's supervision and according to the manufacturers directions.
Many products promise whiter teeth: Whitening toothpastes do not bleach teeth but use mild abrasives and polishing agents which can remove surface stains. Whitening strips are available over the counter and contain a mild bleaching agent which can be moderately effective over an extended period of time. Bleaching trays can be either custom formed or over the counter. Professional trays contain a stronger concentration of bleach and are fitted to optimize tooth exposure. Not all patient's are suitable candidates for bleaching. An examination and cleaning are recommended prior to undertaking bleaching. Results will vary based on the cause of discoloration. It is also important to note that existing restorations, fillings and crowns or bridges do not bleach. Tooth sensitivity may also result from bleaching.
Teeth discoloration may be due to either intrinsic or extrinsic causes. Intrinsic causes are often the result
of pulpa trauma, high levels of fluoride, tetracycline administration during childhood, jaundice in childhood and cavities. Extrinsic staining is mostly from dietary factors : red wine, coffee, tea, carotenes in oranges and carrots and tobacco use. Teeth will also gradually discolor as one ages, from the wearing of the tooth structure or as a consequence of pulpal inflammation. Discoloration due to aging responds much better to bleaching than does discoloration due to tetracylcline.
Patients with generalized gingival recession may experience significant sensitivity. Younger patient's have more porous enamel and wider dentinal tubules. This can also result in greater sensitivity. Old, leaking restorations may allow the solution to irritate the pulp. Should it occur,sensitivity may persist for up to four days after the cessation of bleaching.
